General Health Panel:
CBC (complete blood count), CMP(comprehensive metabolic panel), LIPID (full cholesterol panel), TSH (thyroid stimulating hormone)
THYROID: TSH, T3 Free, T4 Free, TPO, TG ab
HORMONES: Estradiol, Progesterone, PSA, Free and Total Testosterone, Total Testosterone, LH and FSH
Additional Tests offered: Vit D, Folate and B12, HA1C, Iron Panels, Ferritin, Homocysteine, and CRPHS.
Payment is due at time of service. We don’t bill insurance, but upon request can provide an itemized receipt for you to
submit to your insurance company on your own.

• Private / Court Ordered Testing (we can email or fax results as needed)
• Court Ordered Random Testing (varies by court order so we will discuss individual needs)
• Instant 5 panel drug testing
• Lab Based 5 or 10 panels available. Approximate 48-72 hour for results

Paramedical Exams (Life & Disability)
FASTING: 12 hour fast or overnight fasting is best.
Water is recommended to help with the blood draw & to keep you from dehydration.
RESTING EKG'S: No lotions or baby oil should be applied before an EKG, the electrodes need to stick to your skin to get a proper reading of your heart.
APPOINTMENTS: Most requirements from the insurance companies are usually the same and what we need form you takes about 30 to 40 mins.
